CD28
CD28全稱為分化簇28抗原，是T細(xì)胞表面關(guān)鍵的共刺激受體，其晶體結(jié)構(gòu)呈現(xiàn)典型的免疫球蛋白樣二聚體構(gòu)型。該蛋白通過與抗原呈遞細(xì)胞表面的B7分子（CD80/CD86）結(jié)合，為T細(xì)胞活化提供第二信號(hào)，這種雙信號(hào)機(jī)制決定了免疫應(yīng)答的特異性與強(qiáng)度。在腫瘤微環(huán)境中，CD28信號(hào)缺失常導(dǎo)致T細(xì)胞耗竭，而過度激活則與類風(fēng)濕性關(guān)節(jié)炎、系統(tǒng)性紅斑狼瘡等自身免疫疾病密切相關(guān)。目前全球已上市藥物阿巴西普通過競(jìng)爭(zhēng)性結(jié)合B7分子間接調(diào)控CD28通路，用于中重度類風(fēng)濕治療。在CAR-T細(xì)胞治療領(lǐng)域，第二代產(chǎn)品普遍采用CD28或4-1BB作為共刺激結(jié)構(gòu)域，顯著提升了抗腫瘤活性。近年來針對(duì)CD28的新型雙特異性抗體進(jìn)入臨床，既可阻斷病理性的過度激活，又能定向增強(qiáng)抗腫瘤免疫，這種雙向調(diào)節(jié)策略為免疫相關(guān)疾病的精準(zhǔn)干預(yù)開辟了新路徑。
